Nursing and Care Essay Example for Free

Nursing and Care Essay The concept of Synergy can be applied to the practice of nursing by means of education and service. To obtain Synergy in nursing the nurse’s knowledge and skills are able to meet the individual needs of the patient population that is presented. Currently in Western Pennsylvania, there is a rising number maternal substance abuse during pregnancy. This epidemic in turn creates a different type of population in infants than other areas of the Neonatal Intensive Care Unit (NICU), with specific healthcare needs. One area that is lacking synergy is the care of this rising patient population of infants suffering from Neonatal Abstinence Syndrome (NAS). This creates a gap between nursing education and clinical application. Due to the rise in NAS infants it is important for nurses to be educated in proper care for an infant presenting with this diagnosis, symptom management, communication skills, and the fundamental knowledge Neonatal Abstinence Syndrome. Poor patient outcomes can be caused by a lack of understanding addiction. The Vermont Oxford Network states, â€Å"Fear of stigmatization, discrimination, child removal, poor treatment, and criminal prosecution has deterred women from seeking care. However, the research suggests that providing substance dependent women with comprehensive healthcare, drug and alcohol abuse treatment, and social support improves pregnancy, birth, and child development outcomes. † (Horbar, Soll Buus-Frank, 2013) When the infant is admitted into the acute care setting, the care needs of these patients should focus less on the immediate outcome, and more on what will create the greatest good for each patient as an individual. There is an essential need for the development of collaborative solutions for care and education in the healthcare setting for the care of this rising population. Often this would be to provide care with a realistic goal of maintaining optimal levels of comfort with minimal side effects of withdraw. â€Å"About 40% of infants who exhibit withdrawal symptoms can be treated without medication. † (White, 2013) Nurses who take time to ensure a level of comfort will help to alleviate symptoms of NAS withdraw and also help support the family in their infant’s care. In addition to providing comfort will help decrease the need to be medicated for withdraw and ultimately will lead to a shorter hospitalization. The manager of a Neonatal Intensive Care Unit will need to address this issue and implement change in order to improve patient outcomes. The competencies that make up the Synergy model are clinical judgment, advocacy and moral agency, caring practices, facilitation of learning, collaboration, systems thinking, response to diversity, and clinical inquiry. For the NAS population advocacy, clinical judgment, caring practices and facilitation of learning are most important. Advocacy proposes the nurse to serve as a moral agent to work on the infant’s behalf in order to provide the greatest good to help resolve concerns that are both clinical and ethical. Clinical judgment is the ability for the nurse to take on the leadership role to care for their patient. Infants are unable to make healthcare decisions; the nurse must consider the parent’s wishes and what the result the nurses’ actions will yield for this infant. These actions must positively contribute to their plan of care overall. Caring practices creates a compassionate and therapeutic environment for each the patients, recognizing the unique needs of each infant. The nurse acts as a facilitator of learning when he/she incorporates the understanding of the infant’s family into the care. The Synergy mode includes patient characteristics, which are, resiliency, vulnerability, stability, complexity, resource availability, participation in care and decision-making and predictability. For the NAS population the patient characteristics would immediately associate complexity, vulnerability, and participation in care and decision-making. Complexity as referred to as to the multitude of symptoms that are to be considered for proper care of the patient. These infants are unable communicate pain, symptoms needs, and what their desire’s are. For example, these infant’s cry and are inconsolable until the nurse goes through a trial and error until he/she figures out the needs of the child. They do not understand what is happening to them. Vulnerability includes the stressors that these patients will be exposed to that may affect outcomes. The infants depend on the decisions their families and healthcare professionals. Increased length of hospital stay can lead to problems that may decline in their already compromised condition, for example, infection. Lastly, the NAS patients are vulnerable due to the fact that they are unable to make their own healthcare decisions and have to depend on parents and healthcare providers to be speak on their behalf. The parents participate in care as they receive information and help assist in the care for their child. Analyze the Case Nursing education places emphasis on clinical presentation, physical stressors and conditions that affect the patient and their associated treatments. Increased education is needed for nurses on the postpartum and NICU units about the treatment for this specific patient population. With the increase in drug dependent mothers there seems to be little nursing education provided on postpartum and NICU nursing units about the care for these infants, and this issue needs to be addressed. â€Å"Infants with NAS have higher rates of neonatal complications, prolonged lengths of stay and consume substantial NICU and hospital resources. Further, they impose a growing burden on already strained healthcare resources because state Medicaid expenditures are disproportionately impacted. † (Horbar, Soll Buus-Frank, 2013) The administration and education team needs to identify this problem and incorporate teaching and updated research on these units. One competency that lacks synergy in the nursing care of the NAS population is collaboration. â€Å"Collaboration enhances the capacity of a group and increases the potential for success. † (Hardin Kaplow, p. 75, 2005) It is not uncommon when different discipline areas such as the nurse, doctor, social worker etc. identify different goals for the patient. Poor communication causes the team to not work in unison to collaborate and work towards a common goal. â€Å"A key to collaboration is the communication that must exist between the patient, family, and health care team members as well as among members of the health care team. Another synergy lacking competency would be caring practices, which is the unique nursing care provided to the patient and families. Within the NAS population nurses have to recognize the differences and incorporated them into the infants care. Having compassion and creating a calming environment help provide comfort in a therapeutic environment. Often times the NAS population is considered as level II care, this allows for the nurses to take on a multiple patient assignment of three to five infants at a time. The NAS population is unique; they require much coddling and almost constant attention from the nurse. Having greater than three patients causes a great disadvantage to these children due to the nurse having to spread herself thin to meet the basic needs of feeding, changing and medicating, the nurse does not have adequate time to rock and soothe the infants causing greater withdraw symptoms. The rational decision-making model will help identify and implement the appropriate changes that are essential in optimizing care for the NAS population. Improvement in the education of the healthcare staff will create better patient outcomes. This decision making process includes three steps to achieve this objective: identify possible outcomes, determine probability of each outcome, and take action with the highest probability to achieve a positive outcome. The considered actions are increased in-person training, mandatory review of printed materials, or an on-line training course. The greatest positive outcome would come from increased in-person training. Create a Change Proposal Part of the nursing orientation for staff will be to participate in a class specified for the training related to care of the NAS infant prior to working in the NICU. Written material as well as a presentation will be provided. The attending physician will make a statement about services provided to the NAS infant, medications and important facts that the physician would like in report. For the staff nurses’ senior nursing projects are assigned yearly and are presented to maintain their status on the unit. Assigning NAS care to a few nurses each year will keep the most up to date research on the unit. These nurses will participate in Internet presentations that focus on quality, safety and care for infants and families of and NAS infant. Finally, a staff member from the social work team will join and present his/her role in the case of a NAS infant. The unit will provide CEU’s for this training. This proposal will affect the unit internally by involving the management team and multiple staff disciplines. The management team will help aid in this proposal by providing a day that is designated this additional training and appoint the appropriate nursing, physician and social work staff to participate in running the program. Finally obtaining CEU’s approved by the state board of nurses to offer education credits for after the completion of training. The designated nurses assigned to NAS as a senior project must keep up to date with the training information prior to the class of new staff, doing so will ensure they are prepared for questions that may arise. With this proposal the staff has the potential to increase the patient stability, outcome and satisfaction. Also this proposal will be aimed to help the nurses caring for NAS infants to promote patient care through evidenced based research. A change model that applies to this proposal would be the Lewin’s change model. This model is based on unfreezing, moving, and refreezing. First the unfreezing stage, the current beliefs and ideas will change with the desired outcome. The health care team will sufficiently identify the epidemic and need for changes in care for the NAS infant, inspiring the education and treatments needed to move the staff towards bettering practices. Lastly, refreezing, the nursing staff will have a new outlook or attitude towards NAS infants. Changes in the treatment process will be instituted based on the staff’s new outlook. Having the knowledge and resources for the care of the NAS infant will allow the care team to handle each unique case with evidenced based approach. Along with any type of change there will always be positive and negative outcomes on the environment. Positive outcomes will include an influx in nursing knowledge, better patient outcomes and an increased amount of resources for the staff. Negative outcomes may include not having the adequate amount of staff to run this program and the biases formed by individuals prior to participating in this class. A way to follow up with the care provided to these patients and their families would be an optional survey completed by the families allowing the unit to see their strengths and weaknesses. The survey will allow for improvement of care. â€Å"Nurses who have confidence in their ability to positively impact maternal parenting behaviors through education and skill building may impact patient care outcomes. It is imperative that the management and supervisors recognize the issues that nurse’s experience when caring for NAS infants. Educating staff will allow for a balance of practice and the nurses actions. Instituting this educational program will provide synergy to the NICU units and the epidemic of withdraw infants. The unit directors will be able to see that the changes to the unit and education instituted has worked when NAS infants have a shorter hospital stay, a decrease in the use of morphine and phenobarbital and finally patient/parental and nurse satisfaction. In conclusion, with the epidemic of substance abuse mothers on the rise it is imperative that neonatal nursing units properly educate their staff about the treatment and care of NAS infants. With the changes proposed in this case the care of this specific population will be enhanced. These infants are very fragile and require a lot of special attention. The proper care and education could make the world of a difference in a baby’s life.

Documentation and Fabrication in Phonography :: Music Essays

Documentation and Fabrication in Phonography ABSTRACT: In most general terms, my paper is about the mixture of agendas in the recording industry, where documentation, with its apparently educational implications, becomes difficult to distinguish from a range of distinct, even opposed, goals—which I group under the heading "fabrication." After a few historical remarks, I develop the concept of what I call works of phonography (WPs)—that is, sound-constructs created by the use of recording machinery. (Examples: rap music recordings, electronic compositions for tape machine, sonic pastiche's by pop groups such as Art of Noise.) I detail their ontological characteristics, as contrasted the features of ordinary musical works. WPs are—I claim—replete. (Their finest sonic details are constitutive of them.) They are autographic. (Authenticity of their instances is not tested by the allographic criteria we associate with ordinary musical works, namely, compliance with scores.) And they are phono-accessibleâ⠂¬â€that is, accessible only through playbacks of authentic instances of their record artifacts, e.g., cassette tapes, CDs, etc. I then turn to Theodore Gracyk's recent study of rock music (in his book Rhythm and Noise), arguing that his account is formally similar to my account of WPs. This raises the question of whether there be counter-examples to Gracyk's account—particularly of the sort that show his view to be too broad. I bring this to a focus finally by a comparison of rock recordings with jazz recordings—two classes that Gracyk tries to keep ontologically distinct. I argue that many classic jazz recordings are artifacts of the recording studio, no less than those Gracyk identifies as pure cases of rock music. In the same vein, I argue that, once recorded, the improvisational music of jazz is deformed—indeed, that it acquires features of WPs. This has the further implication that Gracyk cannot preserve his sharp distinction between rock and jazz records that he want's to maintain. I. Like Evan Eisenberg, who argued that sound recording has opened up entirely new kinds of musical experience unknown in the age of mere live performance,(1) Ted Gracyk has opened his ears to what Walter Benjamin had to say about mechanical reproduction. Both see sound recording not as a mere convenience but as fraught with broader implications. In his recent book, Gracyk has brilliantly described, not only the phenomenology of rock sound, but how the technology has made possible a type of musical work unknown in the age of mere live music.(2) The recording industry has lived mainly by what might be the called transparency perspective, according to which the analogy for a sound recording is a transparent window pane through which we can view, undistorted, the object of our interest.

Nature and Importance of the Study Essay

Trisales Business Systems Corporation, built for almost 30 years already, is a company engaged in the business of retailing office equipment. Such equipment range from Photocopying machine, Fax machines, down to smaller ones such as calculators and electronic typewriters. Their main suppliers include big names in the appliance industry including Sharp Philippines, Copier Online, Epyon Corporation, Panasonic and Sanyo. TBSC is a small to medium in size. All in all, the company consist of no more than 50 employees, including the staff and the people in the managerial levels. They cater to a number of clients around the Cebu province. TBSC is located in Ramos St., Cebu City. The main aim of sales and stock inventory system is to hold the inventory records accurately and a healthier sales transactions with their costumers and suppliers. Manual recording is really giving them hard time in going thru further for their business. And because they include big names in the appliance industry with a number of clients around the Cebu province, they need to provide an accurate records of their transactions including their inventory as much as well. They also want to have a more secure business flow, and for a well-organized inventory and transaction records. To necessary avoid chaos when it comes to storing information. They believe that having a healthy system for their company is another way of showing their professionalism in the business industry. Statement of the Problem Below specified are the current problems faced by the Trisales Business System Corporation: i) Time consuming transactions and inventory process Without a system, recording transactions and conducting inventory is one of the most difficult and time-consuming job for their company. Their employees are spending most of his/her time jotting down all transactions for the day. By the end of the month, or whenever the company conducts the inventory of stocks, they have to dig through a whole pile of files to determine the number of sales from the previous workdays, to match it with the results of the inventory. ii) Inaccurate inventory of stocks and sales TBSC nowadays is still using the manual recording of informations and data for their business. This causes their records in transactions and stocks to be misguided, filed inappropriately and cluttered. As result, there is difficulty in retrieving data, making analysis and making adjustments to the records. iii) Unsecured records of data and informations The manual system of recording data in TBSC, to be direct to the point, unsecured. Anytime, it may get lost, edited or faded which is more prone to miscalculations and causing incorrect changes to the records. iv) Stocks outs and excesses of stocks in inventory Shortages in the stock inventory can result in product delays to customers which will reduce their expectations to the company. A poor forecasting in the inventory is also leading them in excesses of stocks that results in additional costs to the organization in storage costs and funds tied up in unused stocks. If the company do not use inventory quickly after purchasing, the business will begin to lose money on the materials just because of their slow and unorganized scheme. Objective of the System The main objective of this study is to obtain answers to the questions stated above. We want to know the problems encountered by TBSC employees with regards to their information storing and accessing. Furthermore, we want to demonstrate how the system focuses on those information can be a determinant to improve customer service in Trisales Business Systems Corporation. A proposed solution to the problem: i) To achieve a hassle-free, time bounded transactions and inventory process for the employees in-charge. That would lessen their time in spending on the process of doing transactions and inventory records. Equals more time for doing other productive tasks and to help create a healthier and more productive environment for TBSC. ii) To determine and to minimize the errors in recording transaction and informations. Since TBSC is a growing company, the system will not only stores data, but also does analysis on day to day company transactions. iii) To secured the company important files and records by having a log-in and password boxes as a gatekeeper. Also to prevent from editing forms and important data such as dates and amounts. iv) To control excesses of stock and to prevent stock outs. The system will function to update and arrange the stocks inventory in the appropriate order to give the precise count of their products to prevent excessing and stock outs in further. Significance of the study To the TBSC and Company The planned system significance is to minimize errors and confusion of the employees in recording. Importantly to organize and to give accurate records of the inventory and transactions, for the company to built-up in the industry. Mostly to give stand and to serve the customer’s wants on further. To the fellow researcher The significance of this system is to give a vivid inner view on how the nature of technology will help our career along the way. To embolden researchers who has the same field as mine. There’s no easy way of having this study but it would be the best ever pleasure when that almost everyone in a company is using your programmed system. To the proponents The significance of this study is to support everyone in the field of technology, and to stretch a bit of my knowledge in this field. This will serves as the proponents guide as they embrace the same field as mine. Importantly to give a healthier deal in the business industry. Scope and Limitations Scope of the project The proposed system generally on the sales and stock inventory of the Trisales Business System Corporation. The system purpose is for the regular transactions and operation of their sales and inventory. The proposed system could minimize the problems stumble upon the company records of important data. These are the scope for the proposed system: i) Can build account Employees could have their accounts, especially the person who’s in charge with the sales transactions and stocks inventory. ii) Accumulating Products Through the system, adding new products for the business is more appropriate. The person in charge would just simply input the asked information to the system example: product id, product description, date delivered, pieces ect. The inputted information for the new product will be stored in the database, and could also easily access by the system. iii) Can manage sales transactions The proposed system can manage sales. For example the client requested for a new office supply/product from TBSC. The system will simply ask the user to input some important data from the client and the system will automatically store it. The stored data and information can be used for further transactions with the client. Limitations Below are the possible limits of the proposed program for Trisales Business System and Corporation: Registration in the system is done only by the administrator or the person in charge. Does not support network and internet topology, or online transactions. Storing of informations is limited, only important and the specified information in the system are allowed Multi-tasking could possibly done, but only for the given system for example; Entering data for the new arrived supplies while having the purchasing transaction. Could go through compatibility issues while installing the system. Because of the programmed used in creating the system. The Legalization Of Marijuana For Industrial Use - 940 Words

Power Flower Most citizens of the United States look down upon the thought of cannabis but the legalization of marijuana for industrial use would create jobs, benefit the economy, and contribute to the environment through a number of uses from the plant. A good choice would be to crack down on the sales of marijuana, but the smarter choice would be to legalize! Many jobs would be created for the growth of cannabis. In order for the US to grow and sell marijuana, taxes would be added, therefore creating more money for the economy just by legalizing the growth of a plant. Legalizing cannabis would create an extreme amount of jobs for this growing industry. Dementi A Progressive Disease That Damages Brain - 1492 Words

Dementia is a progressive disease that damages brain cells, wipes out memories, and changes the way a human being understands and functions in the world(Colistoga Press, 2013). When someone is diagnosed with Dementia or Alzheimers, it will change their entire life and their family. When diagnosed early, the patient can help plan their care while they have the ability to do so. The family also has time to plan care for their loved one and research the best option of care for them as they progress in their disease. Although there is not a single test that diagnosis dementia, your healthcare physician can make the diagnosis according to several exams, tests, and review of symptoms. Each person with dementia is different and it affects them differently, and they need to be treated so. It is important to study from the perspective of the family and person who is struggling with dementia. There is no one size fits all when you are dealing with a person who has dementia (Rubinstein, N., 2011). They are unique, and the person you are dealing with is one of a kind, and their relationship with their family is one of a kind, so each person diagnosed with dementia will respond differently to treatment and care. Although dementia does not discriminate, it is found more common in women than in men. It is found mostly in the older population and is rare in younger people. It is a decline in cognitive function usually beginning with memory. Position Paper on Reality for Mathematician- myassignmenthelp

Question: Discuss about thePosition Paper on Reality for Mathematician. Answer: Reality, in the colloquial usage of the term, refers to the state of things that have a physical existence. Philosophers over the years have tried to explain the concept in a varied number of ways from different points of view. In the following paper is attempted a definition of reality that is based on the first and second meditations of Descartes. Rene Descartes, known as the father of modern Western philosophy, was a French philosopher, scientist and mathematician. One of his most celebrated texts, Meditations on First Philosophy published in the year 1641 in Latin, deals with the skeptic analysis of the term reality. The philosopher then tries to establish the beliefs that can be taken to be known with certainty. In the first of the six meditations, is found that the meditator discards all the beliefs that he had faith on during his lifespan till that particular time. The person tries to rebuild his beliefs, this time basing them on more definite arguments (Descartes 2013). In the second meditation, Descartes presents the concept of representationalism. According to the philosopher, a person can reach out to the world in an indirect manner through his own world of ideas, that include all that the mind contains ranging from perceptions, memories and images to beliefs, decisions, intentions and so on. The second meditation con tains the famous theory of Cogito Ergo Sum which translates into I think therefore I am and the wax argument (Hasdan 2017). The theory of Descartes may be objected from a number of varied viewpoints. The theory propounded by the philosopher faces objections regarding the issue of not being able to distinguish between the state of wakefulness and dreams. According to another famous philosopher, Thomas Hobbes, Descartes had identified thinking or intellect with the thing which understands (Adams 2014). Hobbes argues that in the famous argument of Descartes, cogito ergo sum which translates into I think therefore I am [existent], the second part of the proposition depends on the knowledge of the first proposition (Mori 2012). It may be observed that Hobbes had been appearing incoherent in the arguments against the theories of Descartes and is found misquoting them as well. Descartes responds in short replies to all these objections in a rather rude or curt manner. Descartes had replied to the first objection of Hobbes saying that the philosopher had never claimed to introduce novelties with the use of the dream theories, instead it was used to help prepare the minds of the readers for the study of intellectual things. The philosopher, in a response to Hobbess second objection by saying that he had not mentioned the act of understanding to be similar to the thing which understands. In lieu of the discussion, it may be said that Descartes theory of skepticism leads to formation of a mind that questions everything inclusive of its own existence. Reality is inclusive of both the present and the past and does not depend on the comprehensibility and observability of the matter. It does not take into consideration the states in which things might have been imagined or appeared. References Descartes, R., 2013.Meditations on first philosophy. Broadview Press.
